*Job Summary: *Assists the professional nursing staff in providing maximum patient care, assessment, planning, implementation, and evaluation of each assigned patient. Under supervision gives direct and indirect patient care as assignee.


One year of Licensed Vocational Nursing preferred.

Technical Training

IV Therapy course preferred.

*Essential Duties and Responsibilities*

1) Utilizing the nursing process, completes basic physical and psychosocial assessment of patients and takes nursing histories to provide a basis for development of patient care plans under the direct supervision of the licensed professional.

a. Identifies teaching/learning/discharge needs at time of admission and through out hospitalization.

b. Provides a basic assessment which accurately reflects overt physiological and psychosocial needs of patients and reports unusual findings to appropriate licensed personnel.

c. Uses input from patient, family, significant others, physician, and health care team members.

2) Using nursing process, plans and provides patient and family care and evaluates patients' response to care provided and redirects care as necessary under the supervision of a Registered Nurse.

a. Assists with formulating appropriate nursing diagnosis and identifies patient problems from admission data base and ongoing assessment.

b. Establishes patient outcome goals that are realistic, specific, measurable and progressive.

c. Initiates interdisciplinary communications and incorporates recommendations.

d. Consults with a Registered Nurse to formulate care plans that reflect concerns for holistic patient needs for the patient, family and significant others.

e. Assesses and evaluates patient's response and outcome and makes appropriate modifications on the care plan.

f. Reviews and update the care plan as needed with the assistance of the Registered Nurse to reflect patient's current status throughout hospitalization.

3) Implements and monitors appropriate nursing care to meet patient's needs and identifies expected outcome for planned nursing intervention and insures vital information is passed between shifts.

a. Administers medications according to policy, including IV's, IVPB's, and blood transfusions as prescribed by physicians.

b. Knows actions, indications, contraindications, precautions, adverse reactions, dosage and route of administration on all drugs before administering to patient and verifies against medication administration record for accuracy on each shift and on an ongoing basis.

c. Monitors all blood transfusions.

d. Gives exchange report to other health care personnel providing accurate and complete information on each patient at the change of shift, when transferring a patient to another unit, at intervals during the shift.

e. Assesses patients at the beginning of shift for acuity of patient needs/problems and based on that assessment, establishes priority in which work activities are consistently completed by end of the shift.

f. Performs basic nursing skills competently to include but not limited to: intake and output, vital signs, urine testing, specimens/cultures, dressing changes, personal hygiene for patient, skin care, glucose monitoring, airway/suctioning, basic oxygen administration, tube feedings, urinary catherization, drains, enemas, and intravenous catheterization.

g. Notifies supervisor and physicians of emergency situations promptly.

h. Recognizes variances of vital signs and assessments that should be reported to the Registered Nurse, supervisor, and the physician and notify. The physician should be notified in time appropriate to the variance.

i. Functions proficiently in unit nursing modalities.

j. Consistently checks for new patients orders throughout the shift and implements them in an appropriate time frame.

4) Participates in the health care educational needs for the client, family, and others with regard to patient care needs, disease prevention and health maintenance measures.

a. Assist the Registered Nurse in the developing and implementing an individualized teaching and discharge plan at admission and throughout hospitalization.

b. Initiates and documents progressive patient, family, or significant other, and other health disciplines.

c. Act as a liaison between patient, physician, family, significant other, and other health disciplines.

5) Maintains accurate and timely records according to hospital policy and procedure.

a. Accurately documents on each shift the nursing patient care activities and the patient response to nursing interventions and delegated medical management.

b. Assists with maintaining a patient care plan that is current and reflects individual needs and preferences of patients and other(s).

c. Documents teach completed and patient and family response in the patient record.

d. Transcribes and/or cosigns physician orders completely and accurately according to hospital policy.

6) Urgent Care Clinic: Under the direction of a Physician or Physician Assistant provides services and care to clinic clients.

a. Performs selected nursing and administrative duties, and assists physicians as part of the clinical care team;

b. Conducts pre-visit planning by reviewing and updating patient charts prior to appointment with recent test results and correspondence and determines needed services per practice protocol;

c. Confers with physician regarding any incomplete patient tests/consults or other incomplete orders prior to patient visit;

d. Prepares patients for examination and treatment by escorting them from the waiting area to the exam room and taking histories and vital signs;

e. Prepares, cleans and maintains exam and treatment rooms;

f. Gives injections and draws blood; assists with EKGs, OB/GYN exams and office procedures as needed; and provides other patient care services as directed by the physicians;

g. Maintains sample medication records, administers medication upon physician order and responds to medication requests per practice protocol;

h. Prepares and maintains clinical supplies and equipment;

i. Triages and responds to patient communications within practice policy;

j. Assists with scheduling tests and treatments, precertifying patients for hospital care and/or procedures, and processing referrals to other providers;

k. Coordinates care with disease management or case management programs as needed;

l. Contributes to the development of policies and procedures;

m. Participates in professional development activities and maintains professional affiliations;

n. Creates, maintains, copies and files patient files, records and other information as needed;

o. Compiles and condenses technical and statistical data for reports and records;

p. Treats staff, physicians, visitors and patients with dignity and respect.
