St. Christopher's is a progressive social services agency with a 140 year tradition of helping children and families at risk. We seek an experienced and Licensed Practical Nurse to work with our team to provide services to emotionally /behaviorally troubled and learning disabled adolescents at our Residential Treatment Center in New Windsor, NY.

Working as part of a multi-disciplinary team, you will provide acute medical treatment, assessments and pour medication for the youth in care. You will also be required to administer medication and participate in service related meetings that involve the medical aspects of caring for a young person.

*Job Requirements*

Our position requires a LPN certification and a minimum of two years of experience with adolescents/young adults in a hospital residential, or a pediatric setting. Must possess a NYS LPN License.

*I. Job Summary*

The Licensed Practical Nurse (LPN) reports to the Charge Nurse at a Residential Treatment Center Infirmary and is responsible for providing designated health services at a campus Infirmary to patients and emergency medical services to staff as determined by workload or emergency situations.

*II. Essential Functions *

* Pre-pours and administers prescribed and over-the-counter medications to patients under direction of a Registered Professional Nurse, Physician, or Nurse Practitioner.

* Provides prescribed physical care and carries out therapeutic and medical regimens.

* Educates and advises patients, their families as indicated, and Agency staff on health related issues such as sexuality, substance abuse, nutrition, personal hygiene, exercise and medication per Agency and regulatory mandates and per individual patient needs and care plans.

* Takes, reports and records vital signs and measurements and collects specimens as required. Reports abnormal findings to Physician, Nurse Practitioner or RN.

* Assists Physician/NP or RN with Clinics and other health treatment.

* Assists with transport of patients to and from all necessary medical appointments in the event the patient’s medical complexity warrants a nurse’s presence to ensure adequate provision of care.

* Obtains and completes patient histories and medical information on admission.

* Responds to medical emergencies on campus.

* Assists in ordering, storekeeping and dispensing of drugs and medical supplies

* Assists the Medical Social Worker and/or Medical Administrative Assistant in scheduling and coordinating medical appointments.

* Performs various clerical duties such as answering phone calls, directing visitors and filing documents.

* Assists in ensuring a comfortable, orderly, safe and clean environment for patient care.

* Prepares treatment room and assists in stocking and maintaining medical supplies and equipment.

* Presents proper professional appearance at all times; wears appropriate work attire as outlined by Agency Policy. Uses proper phone etiquette and professional interactions with all Agency staff, patients, families and care providers as well as other Health Care Providers.

* Tracks /coordinates healthcare needs of a primary patient case load.

* Provides records and required documents as assigned for Quality Assurance of care purposes.

* Performs other additional related functions as assigned by Charge Nurse.

*III. Job Requirements *

*A. Knowledge: *

· Knowledge of basic first aid principles and techniques.

· Knowledge of cardiopulmonary resuscitation (CPR) principles and techniques.

· Knowledge of policies and practices regarding the dispensation of prescribed and over the counter medications.

· Familiarity with relevant Agency/Departmental health care policies and procedures.

· Knowledge of regulatory, NYS Department of Health and Center for Disease Control mandates.

*B. Physical Skills and Abilities: *

· Must be able to sit, stand and drive a passenger automobile or van.

· Must be able to take and record vital signs and measurements and collect specimens.

*C. Mental Skills and Abilities: *

· Ability to recognize the severity of illnesses and injuries and clearly report information to professional Health Care Providers, provide first aid, and refer patients for additional treatment or summon emergency assistance as necessary.

· Ability to verbally communicate with patients and staff in an effective manner.

· Ability to effectively manage crisis situations involving emotional and/or behavioral needs of patients.

· Ability to accurately calculate and dispense prescribed doses of medications.

*D. Working Conditions: *

· Clinical environment with exposure to infectious diseases.

· Increased risk of contracting infectious diseases such as TB, HBV, HCV and HIV through occupational exposure to high-risk population.

· Exposure to patients with moderate to severe emotional and behavioral disorders.

*E. Education: *

· High School Diploma

· Successful completion of an accredited training course leading to a NYS License as a Practical Nurse

*F. Experience: *

· A minimum of two years’ experience as an LPN

· Experience with the treatment of children and/or adolescents in a clinic or Health Services Department preferred.

*G. Licenses/Certifications: *

· Current NYS License and Registration as a Practical Nurse

· Valid driver license

· CPR/First Aid Certifications

· Possess or be able to obtain Therapeutic Crisis Intervention certification
